Disciplinary Code and Incident Report Form
Student Incident Report FormThe form should be completed by the member of staff most directly involved with resolving or controlling an incident of disturbing behaviour. The form should be completed as soon as possible, usually by the member of staff most directly involved with resolving or controlling an incident of disturbing behaviour.
Examples of incidents, which may cause concern, would include; mental health issues, suicide attempts, and violent behaviour both verbal and physical towards staff. Occasionally other incidents (e.g. police or ambulance attendance) become known to staff that they may feel need reporting. In those cases too, the form can be used.
Disciplinary Code for StudentsThe Disciplinary Code is overseen and monitored by the University Secretary who will usually advise on matters of interpretation and upon the formal stages of the disciplinary process. The Dean of Students' remit includes facilitating mediation and problem solving. Given his close working relations with the University Secretary and the emphasis on informal resolution, the Dean is the prime, initial contact for Colleges and students regarding the Code.
